Enhancing support for moms in our community

Regional One Health’s Women’s Services team is dedicated to helping moms and babies access the care and support they need to thrive. Our new MOM: Maternal One Model Program provides medical and social services for at-risk mothers who visit our Obstetric Emergency Department. By providing regular care and easier access to resources that meet our patients’ individual needs, we can help new moms and babies lead a healthier life.

Our Story

Regional One Health’s innovative MOM Program is a federally funded grant facilitated by the State of Tennessee Department of Health in response to Tennessee’s Healthcare Resiliency Program’s Practice Transformation and Extension arm (HRP-PTE) in the amount of $1, 638,428.31. Our Women’s Services team recognized that many of the patients who seek treatment in our Obstetric Emergency Department are struggling with access to regular medical care and social needs including housing, food insecurities, transportation and more.

We designed the MOM Program to provide intensive support to qualifying moms and their infants by connecting them with medical care at Regional One Health as well as social services provided by partner organizations throughout the community. The MOM program’s success will be rooted in providing CARE (Compassion, Assistance, Respect, and Encouragement). The goal is to provide pregnant patients support from medical professionals and social service agencies to promote a healthy delivery, healthy mom, and healthy baby.

Our Patients

The MOM Program is designed for patients 13 and older who are admitted to the hospital through our Obstetrics Emergency Department. We use specific evidence-based criteria to identify at-risk pregnant patients who meet 3 or more of the following to be eligible for inclusion into the program:

  • Self-pay or no insurance
  • Low-income or under-resourced
  • Positive screenings for social determinants of health
  • Issues including substance use disorder or mental illness
  • Teen pregnancy (13-19 years of age)
  • High-risk occurrences in emergency department or high-risk obstetrical history
  • No prenatal care or late prenatal care

Our Services

When a patient is connected to the MOM program, we start by conducting a comprehensive assessment of their medical, behavioral and social concerns. Then, we design a customized care plan tailored to their unique needs and goals.

Each patient works with a CARE team that includes a Provider Champion, Midwife, Family Nurse Practitioner, Registered Nurse Educator, Social Worker(s) and OB Navigator. Pregnant patients who require high-risk obstetric care will be connected to Regional One Health specialists in maternal fetal medicine. Patients will work with their CARE team to regularly review their plan of care and address new concerns.

The CARE team will stand by each patient’s side for up to a year to make sure moms have the support they need and newborns receive proper pediatric care. Our services include but are not limited to:

  • Childbirth education
  • Prenatal care, health promotion and medication assistance
  • Support for social determinants of health including food, transportation and housing
  • Screening and referrals for postpartum depression and psychosis
  • Support for safety concerns
  • WIC assistance
  • Patient advocacy and wellness checks
  • Childcare assistance
  • Addiction and recovery program assistance

Our Vision

Our goal is to improve health and social outcomes for pregnant women and their infants. By providing regular care, education, and easier access to resources, we can help moms find the support they need, reduce stress and fear, and get the expert prenatal, childbirth and postpartum care that leads to better overall health and quality of life.

This innovative model of care is designed to address health care disparities that contribute to poor outcomes. The MOM program aims to reduce pre-term births, improve infant mortality and maternal morbidity rates, and reduce patients’ reliance on emergency care.

Building authentic, trusting relationships is at the heart of what we do. By giving patients a judgment-free place to seek medical care and support, the MOM program empowers women to build a foundation for a lifetime of better health for themselves and their babies.
